Reason R : Proton $\left(\mathrm{H}^{+}\right)$ always exists in combined form.

Size of nucleus is of order<br/><br/> $1.5 \times 10^{-15} \mathrm{~m}$ or $1.5 \times 10^{-3} \mathrm{pm}$<br/><br/> $\mathrm{H}^{+}$always exists in combined form. <br/><br/>There is no relation between two statements and hence option (D) is the answer.
